eCYBERMISSION Vertebrate Testing Form
According to eCYBERMISSION Rules and Regulations, all teams conducting an experiment involving vertebrates (including humans) must obtain a signed letter of approval from their school’s administration.
The signed approval letter must be faxed to Mission Control at 410-297-2551, as well as submitted as part of the Team’s Mission Folder on the eCYBERMISSION web site no later than Feb. 26, 2010 at 11:59 p.m. ET.

The approval letter should include the following items:
• eCYBERMISSION team name
• Team Advisor name and contact information
• Student usernames
• Name, address and phone number of school (if not on school letterhead)
• Identify the vertebrate animal to be studied
• Describe your project study
• Describe the procedures involving vertebrates
• Describe what you will do with the vertebrate(s) after you have finished your project
• Signature of approval from school administration
Failure to provide this information could lead to a team’s disqualification from the eCYBERMISSION competition.
To help you and your team to comply with this requirement, please contact Mission Control to receive a blank form that your team can utilize when obtaining your school administration’s approval.
